1.) Provide a general definition of Patient-Centered Medical Home (PCMH).
2.) Use web-based and/or other open access resources to identify areas in patient care that require improved focus on quality.
3.) Explain what changes are in-store for primary care providers.
Instruction (Web Quest):
1.) Go to the Agency for Health Research and Quality homepage and click on "Defining PCMH" in the menu bar to learn about the 5 functions and attributes of the Patient-Centered Medical Home.
2.) Go to the the national Committee for Quality and Assurance homepage and click on Patient-Centered Medical Home (PCMH) Recognition. Once you have clicked on that link in the center of the page, you will be directed to the PCMH page. From there, look for 'About PCMH' and select the topic 'Benefits to Clinicians, Practices and Patients'. Document five major benefits.
3.) Watch the following video The future of patient-centered care: Dave Moen at TEDxUMN and take note on Dr. Moen's talk, paying specific attention to his key points about the future of patient-centered care.

Read the two articles provided below and create an outline that depicts your understanding about how the Patient-Centered Medical Home works. Complete this assignment and maintain in a file of your active Population Health work.
